What happened
On February 3, 2020, at approximately 19:00 UTC, an EMBRAER EMB-202A, registration PT-UZW, was performing a private transport flight from an unregistered location in Balsas, Maranhão, to Fazenda Nova Aurora, Maranhão. The flight was intended to facilitate the start of agricultural operations at the destination.
About 1.5 NM from the intended landing site, the aircraft struck the ground. The impact resulted in substantial damage to the aircraft. While the aircraft hit the ground in a controlled manner, characterizing a forced landing, the pilot did not survive the occurrence.
The investigation
CENIPA investigators determined that the aircraft's engine and propeller were producing power at the moment of impact, as evidenced by damage to the engine and the three propeller blades. A thorough technical examination of the powerplant—including the fuel injection system, fuel pump, magnetos, spark plugs, and lubrication system—revealed no mechanical failures, fuel contamination, or maintenance irregularities. The aircraft was within weight and balance limits, and all airworthiness documentation was up to date.
Regarding the pilot, investigators found that he held valid medical and flight certifications and possessed experience in this type of operation. Although the pilot's medical certificate did not list any cardiovascular restrictions, the autopsy revealed that the cause of death was congestive heart failure, pulmonary edema, and hypertrophic cardiomyopathy. The investigation concluded that the pilot likely suffered a sudden medical emergency during the flight, leading to the forced landing.
Findings
- The aircraft was operating without a filed flight plan.
- There was no communication between the aircraft and air traffic control during the flight segment.
- The pilot likely experienced a sudden medical event caused by an undiagnosed pre-existing heart condition.
- Contributing factors included undetermined levels of illness, pain, anxiety, and loss of consciousness.
