What happened
On January 24, 2013, an Embraer EMB-810 C, registration PT-EUP, was conducting a high-value cargo transport flight from Tefé to Flores, Amazonas. While on final approach to runway 11 at Flores Aerodrome (SWFN), the aircraft experienced a loss of power in the left engine. This power loss caused the aircraft to yaw to the left and drift off the approach centerline.
The pilot attempted a go-around to recover the flight path. However, during the maneuver, the crew failed to retract the landing gear and flaps. Consequently, the aircraft was unable to maintain a positive rate of climb. After passing runway 29, the aircraft's left wing struck a cellular antenna located near the traffic pattern. The impact severed a portion of the left wing, after which the aircraft struck a tree and several vehicles in a nearby parking lot. The impact triggered a post-crash fire that partially consumed the airframe.
The investigation
CENIPA investigators examined the engine and its components to determine the cause of the initial power loss. Technical evaluations conducted by an ANAC-accredited workshop under the supervision of SERIPA VII found no evidence of malfunction in any engine component, item, or accessory. The findings indicated that the engine was functioning correctly and that the collision with obstacles occurred while the engine was producing low or no power, despite all systems being in perfect working order.
Due to the intensity of the post-crash fire, investigators were unable to collect fuel samples for laboratory analysis. While the pilots held valid medical and technical certifications, the investigation noted that the maintenance logs for the engine and propeller were outdated.
Findings
- The primary contributing factor was pilot judgment during the execution of the missed approach.
- The aircraft was unable to clear obstacles because the landing gear and flaps remained in the extended position during the go-around.
- An engine power loss occurred during the final approach, destabilizing the landing.
- The aircraft occupants, consisting of two crew members and two passengers, sustained light injuries.
