What happened
On January 26, 2019, at approximately 15:00 UTC, a Hummingbird H1 aircraft, registration PU-FDG, departed from Campo Comandantes Aerodrome (SIJY) in Itajaí, Santa Catarina, for a local recreational flight. Shortly after takeoff, the aircraft lost altitude and struck the ground, resulting in substantial damage to the airframe. The pilot on board sustained light injuries.
The investigation
The investigation by SERIPA V was initiated after authorities were notified of the event by third parties, as the operator failed to report the accident to the competent SIPAER authority. During the inquiry, investigators discovered that the operator had removed the aircraft wreckage from the accident site without authorization from the investigating authority.
Furthermore, the investigation revealed significant regulatory non-compliance. The pilot was operating the aircraft without a valid pilot license or a valid Aeronautical Medical Certificate (CMA). Additionally, while a private sales contract existed between the previous owner and the pilot, the change of ownership had not been updated in the Brazilian Aircraft Registry (RAB). The aircraft's documentation was also not presented to the investigators.
Due to the discovery of intentional illegal acts related to the operation and the fact that the investigation could not proceed effectively due to the removal of evidence, the investigation was officially interrupted.
Findings
- The pilot experienced a loss of directional control following a wind gust immediately after takeoff.
- The pilot was operating the aircraft without the required pilot license or medical certification.
