What happened
On February 17, 2016, a Cessna A188B agricultural aircraft, registration PR-NAS, was performing crop spraying operations near Cruz Alta, Rio Grande do Sul. The aircraft departed from the Cruz Alta Aeroclub (SSAK) to apply agricultural inputs. During a repositioning maneuver, the aircraft experienced a sudden loss of engine power, causing it to collide with high-voltage power lines in the Capão Alto area.
The impact resulted in substantial damage to the aircraft. The pilot, who was the sole occupant, sustained minor injuries.
The investigation
CENIPA's investigation focused on the engine's performance and the legality of the fuel being used. Investigators examined the engine components through analysis conducted by DCTA. The investigation also reviewed the operator's compliance with flight authorization requirements regarding non-standard fuel usage.
Findings
- The engine's fuel injectors for cylinders number 3 and 5 were obstructed, which partially or totally blocked fuel delivery and caused the engine to malfunction.
- The aircraft was certified to operate using AvGas, but was instead operating with ethanol (AEHC).
- The operator did not possess the required Special Flight Authorization (AEV) necessary for using ethanol in an agricultural aircraft equipped with a conventional engine.
- The pilot held valid medical and technical certifications and was experienced in this type of flight operation.
- Meteorological conditions at the time of the accident were favorable for flight.
