What happened
On February 19, 2014, an Embraer EMB-720D, registration PT-FED, was performing a taxi flight from Aeródromo Raposa Serra do Sol (SJMV) to Aldeia Manalai (SJLO) in Uiramutã, Roraima. The flight, operated by TASP Taxi Aéreo Ltda., carried a crew of one and two passengers.
During the landing phase, the pilot encountered a strong tailwind. As the aircraft approached the runway, the pilot utilized smoke near the runway to determine the landing direction. Due to the tailwind, the aircraft's touchdown point was further down the runway than intended, increasing the required stopping distance. To prevent overrunning the runway, the pilot applied heavy braking. This intense braking caused the right tire to burst, leading to a loss of directional control. The aircraft veered to the right and entered a drainage ditch on the side of the runway, resulting in the failure of the left landing gear.
All three occupants escaped the aircraft without injury, though the aircraft sustained substantial damage to the left landing gear and the elevator.
The investigation
The investigation examined the aircraft's weight, maintenance records, and the pilot's decision-making process. Investigators found that the aircraft was operating at a weight exceeding the manufacturer's specified limits. Furthermore, discrepancies were identified between the aircraft's logbook and the maintenance records provided by the inspection company. It was also noted that the logbook had been updated only after the occurrence. The investigation faced difficulties in obtaining precise aircraft data because the operator was in the process of transferring its operational base.
Findings
- The aircraft was operating with a weight exceeding the manufacturer's limits.
- The landing was performed with a significant tailwind, increasing the landing roll.
- The pilot's decision-making regarding braking and landing distance contributed to the tire failure.
- Inadequate flight planning regarding weight and balance.
- Discrepancies existed between the aircraft logbook and official maintenance records.
