Engine failure leads to crash of Embraer Seneca II in Manaus

Casualties unknown • MANAUS, AM, BR

An Embraer EMB 810C struck a house and power lines in Brazil after suffering a left engine failure during takeoff.

What happened

On February 26, 2007, an EMB 810C Seneca II, registration PT-WIG, operated by Amazonaves Táxi Aéreo Ltda, was performing a takeoff from the Flores Aerodrome (SWFN) in Manaus, Brazil. Shortly after departing, the left engine failed. The aircraft experienced a significant leftward yaw and a loss of altitude. During the subsequent descent, the aircraft struck low-voltage power lines and collided with a house under construction near the airfield. The pilot sustained serious injuries, while the passenger suffered minor injuries. The aircraft was destroyed.

The investigation

CENIPA investigators examined the aircraft's performance capabilities and the operational environment. The investigation noted that the aircraft was operating below its maximum takeoff weight, but the configuration used—specifically a 10-degree flap setting—affected the climb performance. Analysis of the engine showed no clear evidence of a mechanical cause for the failure, though investigators found improper maintenance on a fuel pump, which had been inadequately repaired with welding. The investigation also evaluated the aerodrome's safety margins, noting that the runway's end lacked an adequate safety area, with residential structures and warehouses located very close to the runway threshold.

Findings

  • The pilot's inadequate application of flight controls prevented the maintenance of necessary airspeed and configuration to sustain single-engine flight.
  • The pilot's improper judgment regarding the aircraft's ability to maintain flight in a single-engine configuration under the specific weight and configuration present at the time of the failure.
  • The lack of a sufficient runway safety area at the Flores Aerodrome contributed to the pilot's decision to attempt to maintain flight rather than aborting, as obstacles were immediately visible beyond the runway.
  • Inadequate maintenance procedures were identified regarding the aircraft's fuel pump.

Probable cause

The accident was caused by the pilot's failure to properly manage airspeed and aircraft configuration following an engine failure, compounded by the lack of an adequate runway safety area at the airfield.
