What happened
On February 26, 2007, an EMB 810C Seneca II, registration PT-WIG, operated by Amazonaves Táxi Aéreo Ltda, was performing a takeoff from the Flores Aerodrome (SWFN) in Manaus, Brazil. Shortly after departing, the left engine failed. The aircraft experienced a significant leftward yaw and a loss of altitude. During the subsequent descent, the aircraft struck low-voltage power lines and collided with a house under construction near the airfield. The pilot sustained serious injuries, while the passenger suffered minor injuries. The aircraft was destroyed.
The investigation
CENIPA investigators examined the aircraft's performance capabilities and the operational environment. The investigation noted that the aircraft was operating below its maximum takeoff weight, but the configuration used—specifically a 10-degree flap setting—affected the climb performance. Analysis of the engine showed no clear evidence of a mechanical cause for the failure, though investigators found improper maintenance on a fuel pump, which had been inadequately repaired with welding. The investigation also evaluated the aerodrome's safety margins, noting that the runway's end lacked an adequate safety area, with residential structures and warehouses located very close to the runway threshold.
