Bell 206L-3 loses control during low-altitude sightseeing flight in Brazil

Casualties unknown • SÃO BENEDITO, CE, BR

A Bell 206L-3 helicopter crashed in the Serra da Ibiapaba region after encountering a strong wind gust during a low-altitude observation flight.

What happened

On December 28, 2002, a Bell 206L-3 helicopter, registration PT-HTQ, was conducting an aerial observation flight in the state of Ceará, Brazil. The flight originated from Fortaleza, initially destined for Paracuru. During the mission, the pilot diverted the flight path to the municipality of São Benedito at the request of the passengers.

While performing low-altitude maneuvers at approximately 300 feet over the Serra da Ibiap.paba region, the aircraft was flying at a reduced speed to facilitate sightseeing. During a second pass over the area, the helicopter encountered a strong wind gust acting as a tailwind. This sudden change in wind conditions caused the aircraft to lose lift and descend rapidly. The helicopter struck the ground with a high rate of descent, resulting in severe damage to the airframe. Despite the impact, the pilot and the four passengers on board were uninjured.

The investigation

CENIPA's investigation focused on the sequence of events leading to the loss of control and the operational environment of the operator. Investigators examined the pilot's decision-making process, the meteorological conditions at the time of the accident, and the organizational structure of the helicopter school and air taxi company. The investigation also reviewed the aircraft's maintenance history, noting that the airworthiness certificate and recent inspections were valid.

Findings

  • The pilot failed to perform a proper meteorological assessment of the new route following the passenger-requested diversion.
  • The pilot's attention was focused on passenger comfort and sightseeing rather than monitoring critical operational parameters.
  • A strong wind gust (tailwind) caused the loss of lift during low-speed flight.
  • There was a lack of systematic flight training and performance monitoring within the company.
  • The pilot demonstrated overconfidence and reduced situational awareness regarding terrain and wind conditions.
  • The company's management structure lacked adequate oversight of flight operations and pilot training programs.

Probable cause

The accident was caused by the loss of lift during low-speed flight due to a strong tailwind gust, compounded by inadequate flight planning and a lack of situational awareness regarding meteorological changes during a low-altitude sightseeing maneuver.
