What happened
On April 20, 2013, a Piper PA-25-235 agricultural aircraft, registration PT-OPU, was performing pesticide application maneuvers in the rural area of Jaíba, Minas Gerais. The aircraft had departed from the Mocambinho aerodrome (SNMK) for its third takeoff of the day. Shortly after takeoff, the engine experienced a loss of power.
In an attempt to return to the runway, the pilot initiated a left-hand turn. Realizing the aircraft would not reach the landing strip, the pilot attempted an emergency landing in the vicinity of the aerodrome. During the approach, the aircraft struck trees, causing the left wing to separate. The nose of the aircraft impacted the ground, subsequently triggering an explosion and a fire. The pilot sustained serious injuries.
The investigation
CENIPA investigators examined the circumstances surrounding the engine failure and the subsequent impact. The investigation confirmed that the pilot held valid medical and technical certifications and was properly qualified for the mission. The aircraft's airworthiness certificate was valid, and the aircraft was within weight and balance limits.
Maintenance records for the airframe, engine, and propeller were up to date, with all periodic services performed. Additionally, laboratory analysis of a fuel sample taken from the refueling truck showed no signs of contamination. However, the intensity of the post-impact fire and the force of the collision resulted in the total destruction of the aircraft, which prevented a detailed technical analysis of the engine components.
