What happened
On April 16, 2014, at approximately 16:40 UTC, a Piper PA 34-200, registration PT-ISH, was conducting a private flight from Canutama, Amazonas, to Flores, Amazonas. During the flight, the aircraft experienced an engine failure in flight, forcing the pilot to perform an emergency landing at Fazenda Bom Jesus, located in the municipality of Iranduba, Amazonas. The aircraft sustained substantial damage during the landing. While traces of material consistent with blood were found inside the cabin, the specific injury levels of the two occupants could not be determined.
The investigation
CENIPA's investigation revealed a series of critical regulatory violations and operational irregularities. The pilot operated the flight without a filed flight plan and without establishing contact with air traffic control services. Furthermore, the pilot's Multi-Engine Land (MLTE) certificate had been expired since October 2012.
The investigation also established that the aircraft's Airworthiness Certificate (CA) was suspended due to previous damage and an overdue Annual Maintenance Inspection (IAM). Additionally, there was no authorization for the aircraft's ferry flight recorded in the ANAC Civil Aviation System (SACI). The operator failed to report the accident to the relevant aeronautical authorities. Notably, the pilot declined to provide any information to the investigating officer during the inquiry.
