What happened
On April 28, 2018, at approximately 18:40 UTC, a Storm 300 BRG ultralight aircraft, registration PU-JYZ, was performing a flight from the Primavera do Leste Aerodrome (SWPY) to the Querência Aerodrome (SDLN) in Mato Grosso, Brazil. The flight was intended for an ultralight aircraft gathering.
The aircraft began its takeoff roll from the middle of runway 19 rather than the standard threshold. The aircraft became airborne near the end of the runway, but witnesses reported that it lacked sufficient climb performance to clear high-voltage power lines located ahead. The aircraft entered a left turn, lost control, and struck the ground. The impact and subsequent fire destroyed the aircraft and resulted in two fatalities.
The investigation
CENIPA investigators established that the pilot in command did not possess a pilot's license or any valid flight qualification at the time of the accident, although the individual held a valid second-class Aeronautical Medical Certificate (CMA).
Due to the total destruction of the aircraft and all onboard documentation, investigators were unable to verify the validity of the Flight Authorization Certificate or confirm if the aircraft was within its proper weight and balance limits. Furthermore, reliable meteorological data for the moment of the occurrence could not be obtained. The investigation also noted that the aircraft's registration in the Brazilian Aeronautical Registry (RAB) had not been updated to reflect a recent change in ownership.
