What happened
On June 25, 2022, at approximately 19:00 UTC, a Neiva N-592-420, registration PP-XID, was performing a local flight at Arapongas Aerodrome (SSOG) in Paraná, Brazil. The aircraft was carrying one pilot and two passengers. During the landing sequence, the aircraft made an initial touchdown on the runway. Following this contact, the individual at the controls attempted an immediate go-around by advancing the throttle. During this maneuver, control of the aircraft was lost, causing the plane to veer off the left side of the runway. The aircraft then struck irregular terrain, resulting in a rollover. The accident resulted in one fatality and two minor injuries, while the aircraft sustained substantial damage.
The investigation
CENIPA investigators established that while the pilot in command (PIC) was properly licensed and held a valid medical certificate, the person actually operating the aircraft during the landing was a passenger. This individual did not possess a pilot's license, flight rating, or a valid medical certificate. The investigation also noted that the aircraft, operating under an experimental certificate, had undergone undocumented modifications, including the replacement of a door with an acrylic panel. Furthermore, investigators were unable to verify the aircraft's weight and balance or its airworthiness status, as maintenance logs for the engine, propeller, and airframe were not provided.
Findings
- The unlicensed passenger's improper application of flight controls led to the loss of control and subsequent runway excursion.
- The pilot in command demonstrated a failure in risk perception and decision-making by allowing an unqualified individual to operate the aircraft.
- The pilot's attitude was characterized by complacency regarding safety regulations.
- Meteorological conditions at the time of the accident were favorable for visual flight rules.
