What happened
On June 2, 2017, a Piper PA-28-235, registration PT-KEI, departed from the Novo Campo Aerodrome in Boca do Acre, Amazonas, bound for Pauini. The aircraft was carrying a pilot and three passengers. Shortly after takeoff, the engine lost power, forcing the pilot to perform an emergency landing in a grassy area near a local meat processing plant. The impact resulted in substantial damage to the aircraft, while the four occupants sustained light injuries.
The investigation
The investigation, conducted by SERIPA VII, established that the aircraft's Airworthiness Certificate (CA) was suspended because the Annual Maintenance Inspection (IAM) had expired on June 13, 2015. Investigators were unable to verify the maintenance logs for the airframe, engine, or propeller because the pilot and the aircraft owners could not be contacted. The occurrence was not reported by the operator; instead, authorities were notified by third parties and confirmed by the Amazon Air Rescue Coordination Center (SALVAERO-AZ).
