What happened
On May 5, 2018, at approximately 20:30 UTC, a Robinson R44 II helicopter, registration PR-VIA, departed from Jundiaí Aerodrome (SBJD) bound for Vinhedo, Brazil. The flight was intended to transport three passengers and the pilot to a grassy area outside of a registered aerodrome.
During the approach to the landing site, the tail rotor made contact with a building. This impact caused the pilot to lose control of the aircraft, leading to a subsequent impact with the ground. The helicopter was destroyed in the accident. While the pilot and two passengers escaped without injury, one passenger sustained minor injuries.
The investigation
CENIPA's investigation focused on the operational legality and the suitability of the landing site. The investigation established that the operator was performing a specialized aerial service (SAE) but was operating without the necessary authorization for air taxi activities. Furthermore, the operator had not sought the required prior authorization from ANAC for landing at an uncertified location.
Investigators also examined the aircraft's maintenance records and found that the engine and airframe logbooks were not up to date. While the pilot held valid medical and flight certifications and the aircraft possessed a valid airworthiness certificate, the investigation could not verify if the aircraft was within its proper weight and balance limits.
Findings
- The primary cause of the accident was an inadequate approach path characterized by a steep glide slope.
- The landing site was neither certified nor registered, lacking adequate lighting and a proper landing ramp.
- The operator was conducting unauthorized air taxi operations.
- The tail rotor strike with a building was the immediate trigger for the loss of control.
- Maintenance documentation for the airframe and engine was not current.
