What happened
On April 27, 2010, an AT-802 aircraft, registration PR-EBM, operated by the Rio de Janeiro Military Fire Brigade (CBMERJ), was performing a flight sequence at the Resende aerodrome. After departing from runway 26, the pilot executed a series of maneuvers, including a right-hand turn and a subsequent left-hand turn to return to the runway centerline for a low-level pass in the opposite direction.
Following this low-level pass, the pilot initiated a climb with a high pitch angle. As the aircraft crossed runway 26, it entered a steep right-hand bank. During this maneuver, the pilot lost control of the aircraft, which subsequently struck the ground in a street near the aerodrome. The impact sequence involved the aircraft's tailwheel striking a house roof, followed by a collision with a wall and an electrical pole. The resulting impact and subsequent fire destroyed the aircraft and caused damage to nearby buildings and the local power grid. The pilot and one passenger both sustained fatal injuries.
The investigation
CENIPA's investigation focused on the flight profile and the organizational environment of the operator. Investigators examined the aircraft's maintenance records, which showed the engine and airframe were recently inspected. The investigation also noted that the aerodrome was under an active closure notice due to security access issues, though the physical signage marking the runway closure was non-compliant with regulations and difficult for pilots to see.
Analysts reviewed the pilot's experience and the operational procedures of the 2/GOA detachment. The investigation looked into the lack of standardized training programs and the absence of formal operational manuals within the organization. Furthermore, the investigation examined the pilot's decision-making regarding the use of aggressive maneuvers in a densely populated area.
