What happened
On February 20, 2016, at approximately 14:20 UTC, a Cessna 172M, registration PT-JKC, was performing a local flight departing from Fazenda Anahí (SDWU) in Porto Murtinho, Mato Grosso do Sul. The aircraft was carrying a pilot and three passengers.
During the takeoff roll, specifically during the rotation phase, the aircraft experienced a loss of directional control on the ground. The aircraft veered off the left side of the runway and subsequently overturned. Despite the impact, the pilot and all three passengers remained uninjured. The aircraft sustained substantial damage.
The investigation
The investigation by SERIPA IV was delayed because the occurrence was not reported to the SIPAER authority at the time of the event. The investigation only began after the Brazilian National Civil Aviation Agency (ANAC) notified the authorities on May 25, 2016, approximately 95 days after the accident.
Investigators found that the aircraft had been dismantled and transported to Campo Grande, MS, without notifying the investigation authority. This removal of the aircraft and the subsequent alteration of the accident site prevented a reliable investigation into the specific contributing factors of the excursion.
