What happened
On December 14, 2014, at approximately 23:40 UTC, a Robinson R44 helicopter, registration PT-YPW, crashed near Fazenda São João Batista in Buritama, São Paulo. The aircraft was performing a private flight when the crew lost control during the takeoff phase. The helicopter struck the ground approximately 200 meters from the departure point. The impact resulted in substantial damage to the aircraft. Of the two occupants on board, one sustained serious injuries and the other sustained minor injuries.
Near the crash site, investigators found fuel canisters and flyers advertising "Panoramic Helicopter Flights," suggesting the operation was being conducted for sightseeing purposes.
The investigation
CENIPA's investigation focused on the regulatory and operational conditions surrounding the flight. The investigation established that the takeoff occurred at an unregistered location, which did not meet the requirements for a certified helipad. Because the site was not registered, there was no assurance regarding the adequacy of the takeoff surface or the quality of lighting for nighttime operations.
Furthermore, the investigation revealed significant regulatory non-compliance regarding the crew and the aircraft's airworthiness. The investigation was noted to be focused on the fact that the operation was conducted in violation of established aviation laws, which creates latent safety risks.
