What happened
On September 24, 2012, at approximately 08:00 UTC, a Cessna 210L, registration PT-KSB, departed from Aeródromo Pouso da Águia (SWPD) in Cantá, Roraima, Brazil. The aircraft was operated by Paramazônia Táxi Aéreo Ltda. Following takeoff, an airfield employee heard the aircraft crash and observed a fire located just beyond the end of runway 06. The wreckage was located approximately 900 meters past the runway threshold. The impact and subsequent fire resulted in the total destruction of the aircraft and the death of the pilot.
The investigation
CENIPA investigators established several critical irregularities regarding the flight's legality and the pilot's status. The investigation found that the pilot was an escaped inmate from a local penitentiary at the time of the event. Furthermore, the flight was conducted without a filed flight plan or any communication with Air Traffic Control (ATC) services. The investigation also noted that the pilot's medical certificate and single-engine land rating had both expired several months prior to the accident, and the pilot lacked the necessary instrument flight rules (IFR) rating.
