What happened
On March 7, 2008, an EMB 720C aircraft, registration PT-REB, departed from Fazenda Bela Vista in Sena Madureira, Acre, bound for Manuel Urbano. Shortly after takeoff, the pilot experienced significant aircraft vibration and a loss of engine power. In an attempt to regain power, the pilot executed a turn to return to the runway, switched the fuel tank selector to the other wing, and activated the electric fuel pump.
Despite these efforts, the engine continued to fail, making a return to the runway impossible. The pilot subsequently performed an emergency landing in a pasture on a neighboring farm. During the initial phase of the incident, an oil leak was observed near the engine area. Additionally, after draining the fuel tanks, the presence of water in the fuel was discovered. The impact resulted in substantial damage to the aircraft's structure, landing gear, and engine. Of the eight people on board, five passengers and the pilot were uninjured, while two passengers sustained minor injuries.
The investigation
The investigation by CENIPA revealed a widespread lack of regulatory compliance regarding both the aircraft and the crew. The investigation noted that the takeoff occurred from an unregistered and uncertified runway. Furthermore, the aircraft's maintenance records, including the logbook and maintenance control books for the airframe, engine, and propeller, were not provided for review. The investigation also noted that the last maintenance performed by an ANAC-approved workshop was dated December 4, 2006.
