What happened
On 29 May 2019, a Piper PA-12 equipped with floats was performing a flight between two remote locations in Ontario. The aircraft was traveling from an outpost camp situated at Domain Lake toward another camp at Optic Lake. Following the flight, the aircraft was reported overdue at its destination. On the following day, another aircraft spotted the wreckage located near the shoreline of Domain Lake.
During the flight, the aircraft struck the ground, which subsequently triggered a forest fire. The impact resulted in two fatalities, as both the pilot and the passenger were killed. The aircraft was completely destroyed in the accident.
The investigation
The investigation focused on the circumstances surrounding the collision with terrain and the subsequent fire. Investigators examined the flight path between the two outpost camps and the discovery of the wreckage near the shore of the departure point.
