What happened
On March 15, 2021, a Fisher Super Koala ultralight aircraft, registered as C-IEQD, crashed onto the ice of Lake Simcoe near Lagoon City, Ontario. The accident occurred approximately 1 nautical mile from the shoreline. Following the impact, a fire broke out, which completely destroyed the aircraft. The pilot, who was flying alone, sustained fatal injuries in the crash.
The investigation
The Transportation Safety Board of Canada (TSB) deployed a team of investigators to the site of the accident to collect data and assess the circumstances surrounding the event. The investigation focused on the sequence of events leading to the impact on the frozen lake surface.
