In-flight fuel line failure and landing gear fire involving aircraft

No fatalities • Miami-Opa Locka, United States of America • Landing (descent or approach)

A fuel supply line failure led to an engine-related issue and a subsequent wheel fire during the landing phase of a flight.

What happened

During the initial departure phase, a failure occurred within the fuel supply system of the right engine. Specifically, a rubber connector used for the fuel supply line to the right engine oil dilution valve split at the valve located on the aft side of the firewall. This mechanical failure resulted in a condition where the engine was unable to function properly unless the electric boost pump was actively engaged.

As the aircraft returned to its departure airport, a secondary incident occurred following the extension of the landing gear. A fire ignited within the right wheel assembly. As the aircraft performed its landing rollout, smoke began to fill the cockpit, which caused the pilot to lose directional control of the vehicle due to obscured visibility. The aircraft subsequently veered off the runway and came to a stop in a nearby canal.

Findings

Investigation of the wreckage and components revealed that the rubber connector responsible for the fuel line split was in a dry and brittle state. Notably, this specific component showed no evidence of fire damage. Furthermore, the inspection confirmed that there were no other sources of fuel leakage present on the aircraft.

Probable cause

The degradation and brittleness of a rubber fuel line connector caused a fuel supply failure and subsequent engine operational issues.
