What happened
On December 8, 1994, a Robinson R44 helicopter was conducting a training flight near Speyer, Germany. The aircraft departed Speyer at 14:53 local time with two pilots on board, intended for a training session involving several landings. Approximately nine minutes into the flight, while approaching the village of Weingarten from the northeast, the helicopter experienced a mid-air disintegration. The aircraft fell from an altitude of approximately 500 meters, and both occupants were killed in the accident.
The investigation
The investigation, conducted by the BFU, examined the wreckage distribution, which spanned 1.8 km. Significant debris, including the tailboom and parts of the main rotor blade, were found scattered across agricultural fields and near a nursery. The investigation involved international cooperation, including experts from the Robinson Helicopter Company, the NTSB, and the FAA. Investigators analyzed the aircraft's maintenance history, the pilots' flight experience, and meteorological conditions, noting that a warm front was moving through the region with gusty winds.
Findings
While the specific initiating event that caused the main rotor-to-airframe contact could not be definitively identified, the investigation established several contributing factors. The aircraft's design possessed a low margin for error when responding to mechanical failures or pilot errors. Furthermore, the investigation noted that the pilot may not have been sufficiently familiar with the specific handling characteristics of the R44 model required to recover from such an event.
