What happened
On November 17, 2013, at approximately 2:00 p.m., an Air-Cam aircraft, registration N356PR, was involved in an accident at the Angelina Aerodrome in Cotuí, Dominican Republic. The aircraft had departed from the airfield with the intention of returning to Cibao International Airport following a fly-in event. Shortly after takeoff, the aircraft entered a steep turn that exceeded its performance capabilities while at a low altitude and low airspeed. This maneuver resulted in a loss of lift, causing the aircraft to impact the ground near the north side of runway 12 and slide toward the south side of the runway.
The impact resulted in one serious injury to the pilot in command, who suffered a spinal fracture. The accompanying pilot remained uninjured. The aircraft sustained substantial damage, including fuselage deformation, a broken left wing, damage to the nose and tail skid, and the separation of the left main landing gear.
The investigation
The CIAA investigation examined the flight sequence, the mechanical condition of the aircraft, and the crew's actions. Investigators reviewed the pilots' training records, medical certifications, and toxicology results, which were negative for all substances. The investigation also included a physical inspection of the wreckage, checking the control cables, the fuel system, and the engines. The investigation confirmed that the meteorological conditions at the time were favorable for flight operations and that the aircraft's engines and mechanical components were not contributing factors to the accident.
