Water Ingress Causes Amphibious Aircraft to Impact Water in La Romana

Casualties unknown • DO

A Grumman G73T amphibious aircraft experienced a sudden loss of lift and subsequent water impact after structural failure allowed seawater to flood the cabin during takeoff.

What happened

On February 9, 2019, at approximately 4:40 PM local time, a Grumman G73T amphibious aircraft, registration N380GB, was performing a water takeoff from Playa Palmilla in La Romana, Dominican Republic. The aircraft was operated privately and was carrying a crew of two and two passengers.

During the takeoff roll, a significant stream of water was observed exiting the aircraft. Shortly after lifting off, the aircraft entered a gradual descent, impacting the water surface with considerable vertical velocity and a slight rightward tilt. The pilot attempted an emergency landing following the initial loss of control. Despite the impact, all four occupants escaped the aircraft without injury.

The investigation

The CIAA investigation focused on the structural integrity of the airframe and the weight and balance of the aircraft during the takeoff phase. Investigators examined the aircraft's maintenance records, which showed the last annual inspection had been completed in April 2018.

Physical examination of the wreckage revealed that the aircraft's lower and rear structure had suffered significant damage. Specifically, investigators found evidence of structural failure in the right wing float, the right elevator, and the lower fuselage. The investigation also reviewed the pilot's credentials, noting the commander held a valid FAA ATP license with extensive flight experience.

Probable cause

The structural failure of rivets due to water pressure led to rapid water ingress, which increased the aircraft's weight beyond its operational limits and caused a loss of lift during takeoff.
