What happened
On August 18, 2016, a Cessna 172-B, registration OB-584, was conducting an instrument practice flight near the ASIA VOR when the pilot requested to return to Lib Mandi Aerodrome due to sudden abdominal pain.
During the initial approach to runway 32, the pilot reported an unstable approach and requested instructions to perform a go-around and re-establish traffic for a subsequent landing. On the second approach attempt, the aircraft was observed approaching at a high altitude and excessive speed. The pilot continued the maneuver despite the unstable parameters, resulting in a long touchdown approximately 600 meters before the end of the runway.
The aircraft failed to stop within the remaining runway distance, resulting in a runway excursion. The aircraft overran the runway end and struck a sandy mound, which caused the nose gear support to fracture. The impact led to the aircraft capsizing on the sandy surface. The pilot and one passenger were able to egress the aircraft without injuries.
The investigation
The CIAA initiated a field investigation following the occurrence, which included interviews with the pilot and the passenger. The investigation involved coordination with the National Transportation Safety Board (NTSB) of the United States. Investigators also requested airworthiness and maintenance records from the operator and the DGAC. While the final technical report is still in progress, preliminary preventive recommendations have been communicated to the national aeronautical authority.
