What happened
On June 10, 2017, an Antonov AN32, registration HK4833, was performing a non-scheduled cargo flight from La Pedrera to Tarapacá in the Amazonas department of Colombia. During the landing phase on runway 25, the aircraft experienced a lateral runway excursion to the right side. The aircraft came to a stop in the safety area approximately 438 meters beyond the runway threshold.
The aircraft was carrying 45 occupants, including 5 crew members and 4/0 military personnel. While most occupants evacuated the aircraft on their own, one person sustained minor injuries. The aircraft suffered several damages, including a partially fractured nose landing gear, dents to the fuselage, and damage to the right wing and horizontal stabilizer.
The investigation
The GRIAA investigation focused on the operational decisions leading to the flight to Tarapacá. The investigators examined the flight planning, the company's operational manuals, and the crew's approach characteristics. The investigation noted that the flight itinerary had been changed to an aerodrome that was unknown to the operator and was not authorized in its operational specifications.
Investigators also reviewed the aircraft's technical condition and found no mechanical failures or malfunctions contributing to the event. The investigation also addressed concerns raised by the state of design regarding the carriage of 40 passengers on a cargo aircraft, clarifying that the occupants were military personnel transported under specific Colombian civil aviation authorizations.
