What happened
On August 27, 2021, a Zlín Z-526 F, registration OK-ZRD, was performing a demonstration flight as part of a youth flight camp organized by Aeroklub Most z.s. After taking off from runway 20, the pilot initially flew at a very low altitude, approximately 1.5 to 2 meters above the runway, with the landing gear already retracted. Following a brief climb, the pilot executed a 180-degree turn with a steep 60-degree right bank.
As the aircraft continued its flight path toward the village of Braňany, the pilot initiated a steep climb at the end of the runway. At an altitude of approximately 130 meters AGL, the pilot attempted an aerobatic maneuver, specifically a looping turn or snap turn. During this maneuver, the aircraft lost airspeed and entered a wingover followed by a spin. Despite the pilot's attempt to recover the aircraft using control inputs, the low altitude prevented a successful recovery. The aircraft struck a concrete service road at a shallow angle and slid approximately 30 meters across a grassy area before coming to a halt. The impact resulted in two fatalities and the total destruction of the aircraft.
The investigation
The ÚZPLN investigation examined the aircraft's maintenance records, the pilot's qualifications, and the organizational procedures of the flight camp. Technical analysis of the wreckage and the engine showed no evidence of mechanical failure, engine malfunction, or bird strike prior to impact. The fuel was found to be of appropriate quality. The investigation also reviewed the meteorological conditions, which were found to be suitable for VFR flight, and the operational procedures of the camp organizers.
Findings
- The pilot performed the takeoff and flight maneuvers in direct violation of the aircraft's flight manual, including the premature retraction of the landing gear and flaps.
- The pilot performed an unauthorized aerobatic maneuver that was not permitted for an aircraft in the NORMAL category.
- The pilot failed to maintain two-way radio communication with the tower during the flight.
- The flight camp organizers failed to adhere to the established conditions for conducting introductory flights, including a lack of clear safety oversight and the absence of a designated safety supervisor.
- The primary cause of the accident was uncontrolled flight during an unauthorized aerobatic maneuver performed by the pilot.
