ANSV Investigation Summary: Multiple Aviation Incidents in Italy

Casualties unknown • IT

The Italian National Agency for Flight Safety (ANSV) has released findings regarding several aircraft incidents involving various types of aircraft, primarily attributed to human factors.

What happened

Between 2016 and 2018, several incidents occurred involving various aircraft types across Italy. On August 16, 2018, an Alexander Schleicher ASH 26 E, registration D-KZAR, was involved in an incident at Rieti airport. After performing a motorized flight and subsequent thermal soaring, the pilot attempted a landing on RWY 16R using a specific competition-style procedure. During the approach, the pilot failed to adjust flap settings and, upon realizing the aircraft was too high and fast to complete a 180-degree turn to avoid obstacles, attempted a go-around. This resulted in the left wingtip and fuselage striking the ground.

On June 1, 2018, at Alzate Brianza, a Consolidated Vultee Stinson L-5, registration I-AEFE, experienced a left yaw during takeoff. The aircraft veered off the runway and collided with a parked Glaser-Dirks DG 300, registration I-IVPK, causing extensive damage to both aircraft.

On March 18, 2018, a Cessna U 206G, registration D-EVBE, landed at the Massarotti airstrip in Caltagirone. Due to a wet grass runway and sudden turbulence, the aircraft overran the runway and came to rest in a reed bed.

On July 22, 2017, at Novi Ligure, a Consolidated Vultee Stinson L-5, registration I-AEFJ, collided with a parked Sportinė Aviacija LAK-17, registration D-KMOT, while taxiing toward the runway.

On March 5, 2017, a Piper Aircraft PA-28-181, registration I-MAGH, experienced a left yaw following a touch-and-go at the Falcone airstrip, eventually stopping in an adjacent field after the pilot attempted to clear an irrigation ditch.

On May 7, 2016, a Grob Werke ASTIR CS Jeans, registration D-2540, performed an off-field landing near Belluno. Facing strong downdrafts and headwinds, the pilot attempted to pull up to avoid terrain, leading to a tail strike.

The investigation

All investigations were conducted as desk investigations by the ANSV. For the D-2540 incident, onboard data logger information was used to confirm the pilot's account of the descent and wind conditions. For the D-KZAR incident, the investigation relied on the detailed pilot statement provided to the agency.

Probable cause

The primary cause across most incidents was human error, specifically inadequate maneuvering, poor assessment of flight margins, and improper management of the aircraft during critical phases of flight such as takeoff, landing, and taxiing.
