ANSV Investigation Summary: Four Aviation Incidents in Italy

Casualties unknown • IT

The Italian National Agency for Flight Safety (ANSV) has released findings regarding four separate aviation incidents involving light aircraft and gliders.

What happened

On October 6, 2019, a Tecnam P9CT (registration I-9615) was performing a recreational flight near Teglio. While attempting to land on a 350-meter strip parallel to a chairlift, the pilot attempted a go-around after arriving too low. During a right-hand turn to regain altitude, the aircraft struck the chairlift cables, became suspended in the wires, and overturned. The pilot sustained polytraumatic injuries to the legs, while the passenger was unharmed.

On May 24, 2020, a Glaser-Dirks DG-200 glider, registration D-4137, was involved in an accident in Como. After experiencing strong downdrafts while crossing Lake Como, the pilot opted for an off-field landing in a small area. During the final approach, the right wingtip struck a tree, causing the glider to roll and crash into a perimeter wall. The pilot sustained minor abrasions, and the aircraft was destroyed.

On July 20, 2019, an Alexander Schleicher ASG 32Mi glider, registration D-KGIC, performed an off-field landing in a cornfield near Alzate Brianza. The pilot, unable to reach the home aerodrome due to sinking air, chose the field but realized too late that the tall corn would interfere with the landing. Both occupants were hospitalized following the impact.

On June 7, 2012, a Tecnam P92-JS, registration I-RTLF, was conducting a simulated approach at Thiene Airport. Following a tight base and final turn, the aircraft failed to stop within the runway limits, overshooting the threshold of Runway 35 and striking the airport perimeter fence. There were no injuries.

The investigation

The ANSV conducted desk investigations for all four occurrences. For the I-9615 incident, investigators reviewed pilot statements and confirmed no technical failures were present. For the D-4137 glider accident, the investigation examined the pilot's flight experience and the specific characteristics of the chosen landing site. In the case of the D-KGIC, the investigation focused on energy management and the suitability of the landing area. For the I-RTLF incident, the investigation reviewed recent NOTAMs regarding reduced runway lengths at Thiene.

Probable cause

The primary cause across these incidents was human error, specifically involving improper approach management, poor decision-making regarding landing sites, and inadequate execution of go-around maneuvers.
