What happened
On November 9, 2014, at 16:53 UTC, an Aero Boero AB-115, registration PP-GBC, departed from Maringá Airport (SBMG) under Visual Flight Rules (VFR) for a local flight instruction session. The flight, conducted by Aeroclube Regional de Maringá, was intended for Private Pilot training, consisting of one hour of local maneuvers, including touch-and-go landings.
During the first landing attempt, a three-point landing, the aircraft began to deviate from its intended path, drifting toward the left. Both the student and the instructor attempted to correct the aircraft's trajectory, but they were unable to maintain the centerline, resulting in the aircraft exiting the runway. The aircraft sustained light damage, specifically to the right wing tip, while both occupants remained uninjured.
The investigation
The investigation conducted by SERIPA V identified several critical irregularities regarding the training program and operational oversight. It was noted that the aircraft was removed from the scene without authorization from the investigating authority.
Investigators found that the student's flight logs contained a mission designation (PS22) that did not exist within the approved Private Pilot Course Manual. Furthermore, the student had completed more instruction hours than prescribed by the manual without receiving any deficiency ratings, even following the runway excursion. The investigation also revealed that the instructor, while qualified, lacked familiarity with the specific learning levels required by the course manual.
Additionally, the student's flight records lacked detailed comments and showed a lack of oversight from the Course Coordinator. The investigation highlighted a qualitative lapse in the instructional process, where the student was not being properly evaluated for the necessary knowledge levels, and the training exercises did not align with the detailed flight plan requirements.
