What happened
On October 16, 2011, a XAVANTE A-122-A, registration PP-KBU, was conducting a flight instruction mission near Luziânia, Brazil. The flight, operated by Aeroclube de Brasília, initially intended to perform navigation training between SWUZ and SNMH. However, due to adverse weather conditions, the crew—consisting of an instructor and a student pilot—decided to return to the departure aerodrome to perform maneuvers.
Following the completion of training exercises, the instructor took control of the aircraft. While flying over a branch of the Corumbá River, the instructor performed a low-altitude pass. During this maneuver, the aircraft's vertical stabilizer struck an electrical transmission line located approximately six meters above the water. Despite the impact, the flight controls remained functional, allowing the crew to return to the aerodrome and land safely. The aircraft sustained light damage, and both occupants were uninjured.
The investigation
CENIPA investigators examined the flight history and the pilot's professional background. While the instructor was technically proficient, the investigation revealed a pattern of high self-confidence and a tendency toward risky behavior. The investigation also addressed conflicting accounts of the event; while the instructor initially claimed the collision occurred during a simulated engine failure, the student pilot's testimony—which was later corroborated by the instructor—confirmed that the collision happened during an intentional low pass.
Investigators also reviewed the instructor's history at the aeroclube, noting previous instances of flight indiscipline, such as performing low passes during parachute deployment operations, which had previously resulted in disciplinary action.
Findings

Safety action
CENIPA issued a recommendation to the National Civil Aviation Agency (ANAC) to work with the Aeroclube de Brasília to improve accident prevention activities. This includes standardizing procedures, enhancing risk management, and implementing Crew Resource Management (CRM) training to foster a stronger culture of operational safety and flight discipline.
