On May 29, 2009, a US-15LZ 'Kosmos' ultralight aircraft, registration RA-0412G, was involved in a fatal accident in the Sur district of the Ulyanovsk region. The aircraft collided with a birch tree approximately 2 km southeast of the Kezmino settlement.
An inspection of the engine revealed that it remained operational until the moment of impact with the tree. There was no evidence of fire or burning on the aircraft following the collision. Investigators noted that a radio altimeter, which is used to maintain altitude, was not installed in the aircraft.
The pilot, a male born in 1959, sustained fatal blunt force trauma, including a ruptured aorta. A forensic chemical analysis of the pilot's blood showed no presence of ethyl alcohol. At the time of the accident, the pilot was wearing a ZSh-5A helmet, a sweater, a leather jacket, and flight boots.
Two other individuals, identified as a customer and an assistant pilot, were seated in the front seat of the aircraft and were wearing seatbelts at the time of the crash. They arrived at the wreckage site and attempted to check the pilot's pulse, but no pulse was detected. A medical professional from the Kezmino village medical station later confirmed the pilot's death.
