What happened
During an aviation enthusiast rally held at the Pervushino airfield between June 30 and July 2, 2016, an AKM-5 aircraft was being prepared for airworthiness certification. On July 2, 2016, the pilot, who was authorized to perform flight and technical evaluations, operated the aircraft following a request from the owner. The pilot personally prepared the aircraft for flight and, after reviewing the weather conditions with the airfield controller, officially recorded the decision to fly.
The flight included the pilot and one passenger. At 14:15, the pilot took off from the unpaved runway on a heading of 290°. Shortly after takeoff, the airfield controller observed the aircraft lying in the grass to the right of the runway path. The aircraft had experienced a hard landing that resulted in partial structural damage. There was no fire following the incident.
The investigation
According to the airfield controller, the takeoff clearance was granted for a heading of 290°. While the controller was attending to another aircraft, they heard distress calls over the radio and subsequently spotted the aircraft on the ground near the takeoff area. The pilot reported that they had completed all necessary pre-flight preparations independently.
