On September 12, 2021, a Let-410 UVP-E20, operated by LLC Aerokervice, was performing a scheduled passenger flight from Irkutsk airport to the Kazachinskoe settlement. The flight, designated AY51, departed Irkutsk at 13:05 and initially maintained a flight level of FL110.
During the approach to runway 04 at Kazachinskoe, the crew identified fog in the area of the landing site. After failing to establish visual contact with the runway or ground landmarks, the captain initiated a go-around. A second approach to runway 04 was attempted, but the crew aborted that attempt as well due to an unsuitable aircraft position.
The crew then transitioned to a third approach, utilizing runway 22. During this final segment, the aircraft's path was significantly to the right of the extended runway centerline. At 14:51:48, the aircraft, registration RA-67042, collided with obstacles at an altitude of 50 meters (420 meters absolute altitude). The collision occurred 3.1 km from the threshold of runway 22, in an azimuth of 12 degrees.
The accident resulted in four fatalities. Meteorological reports for the time of the accident indicated ground visibility as low as 500 meters with fog in low-lying areas.
