What happened
On January 18, 2015, an S-150, registration RA-1861G, was conducting flight operations at the Borovaya landing site. The pilot, who also owned the aircraft, had submitted a flight plan to Air Traffic Control on January 17, 2015, and arrived at the site before 11:00 local time to prepare the aircraft. The pilot personally handled the refueling and pre-flight preparations.
The initial flight of the day departed at 06:24 local time. This first leg was flown solo by the pilot following a standard circuit pattern around the landing site. Following this flight, a passenger and his wife arrived at the airfield via minibus. The passenger had previously purchased an online certificate for a 20-minute flight.
After the pilot landed the aircraft and boarded the passenger, a second takeoff was performed at 0 07:40 local time. At 08:45, the Nefteyugansk garrison fire service received a report of an aircraft crash in the vicinity of the Kordon recreation center.
The investigation
According to the site's executive director, the pilot appeared to be in good health and showed no signs of alcohol consumption during the morning preparations. The investigation noted that the landing site did not have established procedures for medical screening or meteorological consultations prior to flight. The pilot was responsible for all necessary pre-flight servicing, including fueling.
