What happened
On the evening of 01 March 2021, a private Air Creation GTE Clipper 582 was conducting a demonstration flight near Kangar Main Stadium, Perlis. The flight was intended to promote microlight aviation in the region. After an initial successful takeoff, the pilot observed that the primary landing area at the sports complex had become unsafe due to large crowds of spectators.
Seeking an alternative, the pilot identified an open field near the Royal Customs Complex as a suitable landing site. While the touchdown itself was successful, the aircraft experienced a critical malfunction during the landing roll. The pilot attempted to reduce power and apply brakes, but the engine remained at full throttle. In an attempt to stop the advancing aircraft, the pilot intentionally steered the microlight into a small tree. The aircraft sustained damage to the propeller, engine radiator, landing gear, and wing structure before coming to a halt.
The investigation
AAIB Malaysia examined the mechanical state of the aircraft and the pilot's credentials. Investigators found that the aircraft had no registration number and lacked a valid Certificate of Airworthiness. Furthermore, there was no technical log available to verify that any maintenance had been performed on the aircraft.
Regarding the pilot, the investigation revealed that the pilot's PPL-r license had been expired since June 2006. Additionally, the flight took place on a date that did not align with the authorized NOTAM for the demonstration event.
Findings
- The primary cause of the incident was a dented throttle cable, which was likely struck by an outstretched rope on the field during the landing roll. This dent caused the throttle paddle to fail, preventing the pilot from reducing engine power.
- The aircraft was not registered with CAAM and lacked the necessary permits to fly.
- The pilot was operating with an expired license and without a valid medical record.
- There was a lack of documented maintenance practices, leaving the airworthiness of the aircraft unverified.
- The flight was conducted outside of the officially authorized window specified in the NOTAM.
