What happened
On April 3, 1997, at approximately 06:50Z, an EMB-810D Seneca, registration PT-RXQ, was performing a ferry flight from São Miguel do Araguaia, GO, to Goiânia, GO. The pilot had been conducting engine checks at the runway threshold for approximately ten minutes prior to departure.
During the takeoff roll, immediately after rotation, the pilot experienced a heavy impact on the left wingtip. While in flight, the pilot inspected the aircraft and identified significant damage to the wing structure. The pilot subsequently decided to return to the airfield and performed a landing on the opposite runway. Upon inspection on the ground, the pilot discovered a bicycle and the body of a person on the runway. The collision had resulted in one fatality involving a third party.
The investigation
CENIPA's investigation focused on the operational environment of the São Miguel do Ar/aguaia airfield. The investigation established that the airfield lacked adequate perimeter fencing and warning signage to alert pedestrians or animals to aircraft operations. Furthermore, the airfield lacked a designated administrator or any established emergency procedures.
The investigation noted that the cyclist had been attempting to cross the runway via a dirt path through tall vegetation. This vegetation obscured the cyclist's approach, making it impossible for the pilot to detect the person before the impact occurred. The investigation also confirmed that the pilot was properly qualified, the aircraft maintenance was up to date, and the pilot had correctly selected a runway heading to avoid sun glare.
Findings
