What happened
On January 24, 2003, an EMB-201 A Ipanema, registration PT-GYR, operated by DDA Aviação Agrícola Ltda., was involved in a ground accident at Granja S. Gabriel in Jaguarão, RS. The aircraft was engaged in agricultural spraying operations. Following a landing, the crew attempted to reposition the aircraft on the unpaved runway to align for takeoff.
During the process, the ground crew encountered a mechanical issue with a locked steering caster. A ground worker, who lacked professional qualification for aircraft maintenance, attempted to repair the component. After the repair and subsequent refueling, the pilot started the engine. Because the aircraft could not be turned using its own steering system, the pilot requested ground personnel to manually push the aircraft. While attempting to assist colleagues on the opposite wing, the worker walked directly in front of the aircraft and was struck by the moving propeller. The impact resulted in the amputation of the worker's left hand and wrist and injuries to the right elbow. The pilot immediately shut down the engine and provided assistance.
The investigation
CENIPA's investigation revealed that the aircraft was being operated in a high-pressure environment, as the operator's fleet was largely grounded due to maintenance issues. The investigation established that the pilot, while experienced, failed to perform a safety briefing (brifim) for the ground crew and made the poor judgment decision to operate the engine while personnel were actively maneuvering the aircraft.
Furthermore, the investigation found that the ground crew had not received specific training for these maneuvers, and there were no standardized procedures for such operations. The investigation also noted that the operator failed to report the accident to the proper authorities at the time of the occurrence.
