What happened
Following a period of inactivity lasting since June 1989, the aircraft performed two high-speed taxi tests prior to its departure. During the takeoff sequence, the flight crew contacted air traffic control to request an immediate return to the airport for landing.
As the aircraft was observed on a downwind pattern at an altitude between 200 and 800 feet, it maintained a nose-high attitude of approximately 15 to 25 degrees. During this phase of flight, the aircraft entered a stall. The impact resulted in two fatalities.
Findings
Investigation into the mechanical state of the aircraft revealed that the compressor surge valves for both the left and right engines were positioned in the open state. This mechanical condition was identified as a primary factor in the loss of control.
