What happened
On 2D January 2022, a DJI Phantom 4 Advanced with registration ZT-UEE was being utilized for security surveillance at Rooikransberg Farm, located near McGregor in the Western Cape. The operation was being conducted under visual line of sight (VLOS) in accordance with Part 101 regulations.
The flight commenced at 1459Z with a full battery charge. After traveling approximately 2 kilometers from the launch point, the aircraft was returning to its takeoff location when it struck the top of a tree measuring 62 feet in height. At the moment of impact, the battery level was at 87%. The collision resulted in substantial damage to the aircraft, specifically involving the breakage of one motor mounting arm and damage to all four rotor blades.
The investigation
SACAA AIID examined the circumstances surrounding the flight and the credentials of the operator. The investigation confirmed that the pilot held a valid Remote Pilot Licence with a VLOS rating and a current Class 4 medical certificate. The operator possessed a valid Remotely Piloted Aircraft System (RPAS) certificate, and the aircraft had undergone a maintenance inspection earlier that year in January. The weather conditions during the flight were noted as fine, and the mission had been authorized by the farm owner.
Findings
- The primary cause of the accident was that the pilot lost concentration during the return leg of the flight.
- The aircraft struck an obstacle (a treetop) while navigating back to the takeoff point.
- The drone was operating under established regulatory frameworks and possessed valid documentation and inspections.
