What happened
On the morning of 9 August 2022, a Windlass Aquilla microlight, registration ZU-EKY, attempted a takeoff from a gravel road on Kaalfontein Farm in the Victoria West District of the Northern Cape. The flight, conducted under Part 94 regulations, was intended to be a local flight around the district for a hunting excursion.
Prior to the flight, the pilot experienced difficulty starting the engine in the 3°C morning temperatures. After successfully starting the engine and monitoring parameters, the pilot commenced the takeoff roll along the farm's dirt road. Shortly after becoming airborne at an altitude of approximately one meter, the aircraft experienced a loss of lift on the right wing, causing it to drift to the right. In an attempt to maneuver away from a telephone pole, the pilot turned further to the right, resulting in a loss of altitude. The nose gear struck the ground, causing the aircraft to flip over.
The passenger managed to exit the wreckage and assisted the injured pilot. Local assistance was provided by a nearby farmer, and medical personnel from De Aar eventually stabilized the pilot before he was airlifted to a hospital in Kimberley and later transferred to Gauteng for continued care. The aircraft suffered substantial damage.
The investigation
The SACAA AIID investigation confirmed that the aircraft was within its maximum takeoff weight at the time of the incident. The investigation also verified that the pilot held a valid National Pilot Licence and medical certificate, and that the aircraft had a valid Certificate of Registration and Authority to Fly. Records showed the aircraft had undergone its last annual inspection earlier that year and was within its service limits.
