What happened

On the morning of 15 January 2023, a Piper PA-30-160 Twin Comanche, registration ZS-EAW, was conducting a multi-engine conversion training flight out of Grand Central Aerodrome (FAGC) in Gauteng. The flight, operated under Part 141 training regulations, initially proceeded with several touch-and-go landings. After completing three circuits, the crew requested to divert toward the Johannesburg South General Flying Area.

While flying at approximately 600 feet above ground level near the Crown Mines dump site, an eyewitness observed the aircraft's engines cease operation. The aircraft descended rapidly and impacted the base of the mine dump near the National Recreation Centre. The impact resulted in the destruction of the aircraft and two fatalities.
