What happened
On 19 March 2000, a solo gliding operation at the Bloemfontein Gliding Club resulted in a fatal accident. The aircraft, an Olympia EON 465 with registration ZS-GGL, was being launched via a winch system. The winch utilized two engine-driven spools, with a mechanism used to select between the left and right spool.
Earlier that day, the first two launches were successful, though the second launch experienced a mechanical issue where the winch selection lever dislodged, cutting power to the left-hand spool. At an altitude of roughly 200 to 300 feet, the pilot opted to perform a low-altitude right-hand circuit to land on runway 27 rather than landing straight ahead. Following this, a third launch proceeded without incident.
During the fourth launch, the same mechanical failure recurred. As the aircraft reached an altitude of approximately 200 to 300 feet AGL, the selection lever disengaged from the left-hand spool. Witnesses observed the pilot release the cable and deploy the air brakes. Subsequently, the aircraft entered a right-hand bank that increased until the glider entered a spin. The aircraft completed one and three-quarter turns in a near-vertical, nose-down attitude before striking the ground. The pilot sustained one fatality.
The investigation
Investigators examined the winch mechanics and the flight sequence leading to the impact. The inquiry focused on the repeated failure of the winch selection lever and the aerodynamic state of the Olympia EON 465 during the final moments of the flight. The investigation also reviewed the pilot's decision-making during the previous launch failure earlier in the day.
