What happened
On 3 November 2001, a Reims F406 operated as a commercial cargo flight departed Johannesburg International Airport (FAJS) at 0119Z. The flight was destined for Eros Airport in Windhoek, Namibia, carrying two pilots, one passenger, and a significant load of express freight parcels along with two heavy steel bars.
Approximately 106 seconds after the takeoff roll began, the aircraft struck the ground in a marshy area located roughly 700m south of the runway 03R threshold. The impact occurred with the left wing in a low attitude, leading to the total destruction of the aircraft. All three people on board were fatally injured.
The investigation
SACAA AIID investigators examined the aircraft's weight and balance, cargo configuration, and maintenance records. The inquiry established that the aircraft was significantly over its maximum allowable weight, specifically by approximately 699.6kg, representing a 16% overload.
Furthermore, the investigation found that the cargo, including the heavy steel bars, had not been properly secured for flight. The passenger on board also lacked a dedicated seat or any form of restraining device. Regarding maintenance, the aircraft's Certificate of Airworthiness was invalid at the time of the accident, as both engines had exceeded their required time between overhaul (TBO) by roughly 185 hours.
