What happened
On 03 February 2002, a Fournier FR4D, registration ZS-UEM, was involved in a takeoff accident at the Deneysville Yacht Club private aerodrome. The pilot was conducting a private VFR flight, intending to proceed to Springs Aerodrome (FASI).
During the takeoff roll, the pilot attempted the departure at an airspeed of approximately 30 mph without the use of flaps. Before the tail of the aircraft could lift, the pilot lost directional control, resulting in a ground loop. The force of the maneuver caused the undercarriage to collapse and resulted in significant damage to the propeller.
The investigation
Investigators examined the aircraft's condition and maintenance history following the incident. At the time of the accident, the aircraft's mass and balance were within established limits. The pilot weighed 70 kilograms, and the aircraft was carrying 20 liters of remaining fuel.
Maintenance records indicated that the aircraft had been properly maintained according to scheduled inspections. The most recent annual inspection had been completed on 10 May 2001. At the time of the occurrence, the airframe had accumulated 946 total hours, while both the engine and propeller had flown 45 hours since their last overhaul.
Findings
