What happened
On 24 January 2014, a Cessna A188 Agwagon, registration ZS-NDY, departed Wonderboom Aerodrome (FAWB) on a ferry flight intended for Warmbaths Aerodrome (FAWA). The flight followed a mandatory periodic inspection and was conducted under visual flight rules. After an uneventful takeoff, the aircraft deviated approximately 2 nautical miles from its planned route, flying at a very low altitude near the pilot's family residence in the Petronella area.
An eyewitness observed the aircraft flying at roughly 30 meters above the ground, nearly striking a tree. The pilot then executed a vertical, nose-up maneuver, characteristic of aerobatic flight. Following this pitch, the aircraft nose-dived and impacted the ground with significant force. The pilot sustained fatal injuries and was pronounced dead at the scene. The aircraft was destroyed upon impact.
The investigation
SACAA AIID investigators examined the wreckage and the pilot's records to determine the cause of the crash. The investigation confirmed that the pilot held a valid private pilot license and was rated for the aircraft type. The aircraft's maintenance logs, including the recent periodic inspection, were found to be in order, and the engine and airframe showed no evidence of mechanical failure or structural defects prior to the accident.
Investigators also reviewed the meteorological conditions, which were clear with no significant weather impediments. The aircraft was equipped with standard navigational instruments, though a portable GPS was not in use during the flight. The wreckage analysis showed the aircraft hit the ground in a nose-down, wings-level attitude, and the impact was severe enough to destroy the cockpit and engine components.
