What happened
On the morning of 3 May 2021, a Magni M16 gyrocopter, registration ZU-BDY, departed Laingsburg Airfield in the Western Cape for a scenic flight to Morning Star Airfield. The flight was being conducted under visual flight rules as a private operation. Shortly after takeoff, while en route, the aircraft's engine suddenly ceased operation, causing a total loss of thrust and altitude.
The pilot attempted to execute an emergency forced landing, but the aircraft struck the ground at Baviaan Weg farm in a steep, nose-down attitude. The impact was severe, and a significant post-impact fire erupted, consuming the wreckage. While the pilot managed to crawl from the cockpit with serious injuries, the passenger sustained fatal injuries.
The investigation
An investigation by the SACAA AIID focused on the circumstances surrounding the engine failure and the subsequent crash. Investigators examined the wreckage, noting that the aircraft impacted the ground on its right side. Although the engine remained largely intact, the fire destroyed the cockpit and various engine components, such as rubber pipes and belts.
Evidence from the site confirmed that the aircraft had sufficient fuel at the time of the accident. However, the investigation faced significant hurdles. The investigators noted that the pilot had not submitted the required accident questionnaire and that a person responsible for previous repairs to the aircraft was also not cooperating with the inquiry.
Findings
Because the aircraft was not maintained in accordance with the manufacturer's prescripts or Civil Aviation Regulations, the investigators were unable to perform specific tests or research. Consequently, the investigation could not establish definitive findings, causes, or contributing factors regarding the mechanical nature of the engine failure. The investigation was ultimately hindered by the lack of cooperation from involved parties and the non-adherence to maintenance standards.
