What happened
On July 3, 2022, at 07:30 UTC, an Evektor SportStar RTC, registration TC-UFN, operated by Fenix Havacılık A.Ş., experienced a serious incident during takeoff from runway 07L at Bursa Yenişehir Airport (LTBR). Following takeoff, the left main landing gear wheel detached from the aircraft. The crew became aware of the issue after another pilot on the ground alerted the tower. The pilot performed a low pass to verify the missing wheel and subsequently executed a single-wheel landing procedure. There were no fatalities or injuries, though the aircraft sustained light damage to the left landing gear and braking system.
The investigation
The UEİM investigation examined the aircraft's maintenance history and the circumstances surrounding a recent tire replacement. Records showed that on June 30, 2022, a work order was opened to replace the left main tire. The investigation established that while the maintenance manual required a specific 4x40 mm split pin to secure the axle nut, this specific part was unavailable in the company's inventory. Consequently, a technician used a smaller-diameter split pin from the warehouse instead. The investigation also reviewed the pilot's licenses, medical certificates, and the aircraft's weight, balance, and fuel status, finding all within legal limits.
