What happened

On 14 March 2017, an S-92A helicopter, registered as EI-ICR, was involved in an accident at Black Rock, Co. Mayo. The investigation into the circumstances of this occurrence remains active.

The investigation

Following the release of a draft final report in September 2019, the investigation entered a period of formal review. During the comment period for the draft report, a party requested a re-examination of specific findings and a conclusion contained within the document. This request was made under Regulation 15 of SI 460 of 2009.

In response to this notice, the Minister for Transport, Tourism and Sport appointed a Review Board on 23 December 2019 to conduct a formal re-examination of the investigation's results. The AAIU has notified all relevant interested parties of the contact details for the Departmental body overseeing this review.

As the work of the Review Board is currently ongoing, the AAIU has not yet released the final conclusions regarding the accident.
