What happened
On 24 January 2025, a Beechcraft King Air B200, registered ZS-SSP, was operating a commercial charter flight from Winterveldt Mine Airport to Pilanesberg International Airport. The flight, which included a pilot, co-pilot, and three passengers, was being conducted under instrument flight rules in clear weather conditions.
Approximately 38 minutes into the journey, while cruising at flight level 220, the aircraft's door unexpectedly unlocked and swung fully open. This caused an immediate and rapid decompression of the cabin. The crew promptly declared an emergency and initiated a descent to flight level 090. Due to uncertainty regarding whether the door remained securely attached to the fuselage, the crew diverted to Lanseria International Airport. The aircraft landed safely at 1451Z, and although the aircraft sustained minor damage, there were no injuries to the five occupants.
The investigation
An investigation by the SACAA AIID established that the incident occurred roughly 40 nautical miles east of Pilanesberg International Airport. During the flight, the opening door caused secondary damage to one of the rear right-side windows, which was struck by the staircase handrail chain strap.
Investigators confirmed that the aircraft's maintenance records were up to date, with the last major inspection completed in August 2024. The crew followed emergency protocols, and while fire and rescue services were placed on standby at the diversion airport, they were not required upon the aircraft's arrival. The aircraft was taxied to its parking position with the door still open.
